1. Charles de Gaulle Airport Terminal 2E Disaster:
Paris Charles de Gaulle Airport is the largest international airport in France and the second largest in Europe. On 23 May 2004, shortly after the inauguration of terminal 2E, a portion of it collapsed near Gate E50, killing four people. Two of the dead were reported to be Chinese citizens, one Czech and the other Lebanese. Three other people were injured in the collapse. Terminal 2E had been inaugurated in 2003 after some delays in construction and was designed by Paul Andreu. Administrative and judicial enquiries were started. 2. R101 Airship Disaster:
R101 was one of a pair of British rigid airships completed in 1929 as part of a British government programme to develop civil airships capable of service on long-distance routes within the British Empire. It was designed and built by an Air Ministry-appointed team and was effectively in competition with the government-funded but privately designed and built R100. When built, it was the world's largest flying craft at 731 ft (223 m) in length, and it was not surpassed by another hydrogen-filled rigid airship until the Hindenburg flew seven years later. 3. Bhopal Disaster:
The Bhopal disaster, also referred to as the Bhopal gas tragedy, was a gas leak incident on the night of 2–3 December 1984 at the Union Carbide India Limited (UCIL) pesticide plant in Bhopal, Madhya Pradesh, India. It is considered to be the world's worst industrial disaster. Over 500,000 people were exposed to methyl isocyanate (MIC) gas. The highly toxic substance made its way into and around the small towns located near the plant. Estimates vary on the death toll. The official immediate death toll was 2,259. The government of Madhya Pradesh confirmed a total of 3,787 deaths related to the gas release. More details
4. Blackout Disaster:
The Northeast blackout of 2003 was a widespread power outage throughout parts of the Northeastern and Midwestern United States, and the Canadian province of Ontario on August 14, 2003, beginning just after 4:10 p.m. EDT. Most places restored power by midnight, some as early as 6 p.m. on August 14. New York subways resumed limited services around 8 p.m. Full power was restored to New York City and Toronto on August 16. At the time, it was the world's second most widespread blackout in history, after the 1999 Southern Brazil blackout. 5. Hindenburg Disaster:
The Hindenburg disaster occurred on May 6, 1937, in Manchester Township, New Jersey, United States. The German passenger airship LZ 129 Hindenburg caught fire and was destroyed during its attempt to dock with its mooring mast at Naval Air Station Lakehurst. There were 35 fatalities from the 97 people on board (36 passengers and 61 crewmen), and an additional fatality on the ground. The disaster was the subject of newsreel coverage, photographs, and Herbert Morrison's recorded radio eyewitness reports from the landing field, which were broadcast the next day. A variety of hypotheses have been put forward for both the cause of ignition and the initial fuel for the ensuing fire. More details

7. Springhill Mining Disaster:
Springhill mining disaster may refer to any of three Canadian mining disasters that occurred in 1891, 1956, and 1958 in different mines within the Springhill coalfield, near the town of Springhill in Cumberland County, Nova Scotia. The mines in the Springhill coalfield were established in the 19th century, and by the early 1880s were being worked by the Cumberland Coal & Railway Company Ltd. and the Springhill & Parrsboro Coal & Railway Company Ltd. These entities merged in 1884 to form the Cumberland Railway & Coal Company Ltd., which its investors sold in 1910 to the industrial conglomerate Dominion Coal Company Ltd. (DOMCO). More details
8. Chile Mining Disaster:
The 2010 Copiapó mining accident (Chilean mining accident), began on Thursday, 5 August 2010, with a cave-in at the San José copper–gold mine, located in the Atacama Desert 45 kilometers (28 mi) north of the regional capital of Copiapó, in northern Chile. Thirty-three men, trapped 700 meters underground and 5 kilometers from the mine's entrance via spiraling underground ramps, were rescued after 69 days. After the state-owned mining company, Codelco, took over rescue efforts from the mine's owners, exploratory boreholes were drilled. Seventeen days after the accident, a note was found taped to a drill bit pulled back to the surface: "We are well in the shelter, the 33 of us". More details
9. Eschede Train Disaster:
The Eschede derailment occurred on 3 June 1998, near the village of Eschede in the Celle district of Lower Saxony, Germany, when a high-speed train derailed and crashed into a road bridge. 101 people were killed and 88 were injured. The cause was a single fatigue crack in one wheel that, when it failed, resulted in a part of the wheel becoming caught in a set of points, effectively changing the setting of the points whilst the train was passing over it. This led to the train's carriages going down two separate tracks, causing the train to derail and crash into the pillars of a concrete road bridge, which then collapsed and crushed two coaches. 11. Milwaukee Water Disaster:
The 1993 Milwaukee Cryptosporidiosis outbreak' was a significant distribution of the Cryptosporidium protozoan in Milwaukee, Wisconsin, and the largest waterborne disease outbreak in documented United States history. The Howard Avenue Water Purification Plant (see Town of Lake water tower) was contaminated, and treated water showed turbidity levels well above normal. It was one of two water treatment plants for Milwaukee. The root cause of epidemic was never officially identified; initially it was suspected to be caused by the cattle genotype due to runoff from pastures. It was also thought that melting ice and snowmelt carrying Cryptosporidium may have entered the water treatment plants through Lake Michigan. More details
12. PEPCON Disaster:
On May 4, 1988, a fire followed by several explosions occurred at the Pacific Engineering and Production Company of Nevada (PEPCON) chemical plant in Henderson, Nevada. The disaster caused two fatalities, 372 injuries, and an estimated $100 million of damage. A large portion of the Las Vegas Valley within a 10-mile (16 km) radius of the plant was affected, and several agencies activated disaster plans. The PEPCON plant, located in Henderson, Nevada, 10 miles (16 km) from Las Vegas, was one of only two American producers of ammonium perchlorate ("AP"), an oxidizer used in solid propellant rocket boosters, including the Space Shuttle, military weapons and non-weaponized rocket programs. 13. Times Beach Disaster:
Times Beach is a ghost town in St. Louis County, Missouri, United States, 17 miles (27 km) southwest of St. Louis and 2 miles (3 km) east of Eureka. Once home to more than two thousand people, the town was completely evacuated early in 1983 due to TCDD—also known as dioxin—contamination. It was the largest civilian exposure to this compound in the history of the US. In 1985, the State of Missouri officially dis-incorporated the city of Times Beach. The site of Times Beach now houses a 419-acre (170 ha) state park commemorating U.S. Route 66—the famous highway that stretched from Chicago, Illinois, to Santa Monica, California, and passed by the community—as well as the history of the Times Beach area. 14. Skylab Disaster:
Skylab was the first United States space station, launched by NASA, occupied for about 24 weeks between May 1973 and February 1974. It was operated by three separate three-astronaut crews: Skylab 2, Skylab 3 and Skylab 4. Major operations included an orbital workshop, a solar observatory, Earth observation, and hundreds of experiments. Unable to be re-boosted by the Space Shuttle, which was not ready until 1981, Skylab's orbit decayed and it disintegrated in the atmosphere on July 11, 1979, scattering debris across the Indian Ocean and Western Australia. As of 2020, Skylab was the only space station operated exclusively by the United States. More details
15. Sunset Limited Train Disaster:
The Big Bayou Canot rail accident was the derailing of an Amtrak train on the CSX Transportation Big Bayou Canot Bridge near Mobile, Alabama, United States, on September 22, 1993. It was caused by displacement of a span and deformation of the rails when a tow of heavy barges collided with the rail bridge eight minutes earlier. 47 people were killed and 103 more were injured. To date, it is both the deadliest train wreck in Amtrak's history and the worst rail disaster in the United States since the 1958 Newark Bay rail accident in which 48 lives were lost. More details
16. Software Disasters:
On June 4th, 1996, the very first Ariane 5 rocket ignited its engines and began speeding away from the coast of French Guiana. 37 seconds later, the rocket flipped 90 degrees in the wrong direction, and less than two seconds later, aerodynamic forces ripped the boosters apart from the main stage at a height of 4km. This caused the self-destruct mechanism to trigger, and the spacecraft was consumed in a gigantic fireball of liquid hydrogen. The disastrous launch cost approximately $370m, led to a public inquiry, and through the destruction of the rocket’s payload, delayed scientific research into workings of the Earth’s magnetosphere for almost 4 years. 17. MV Bright Field Disaster:
MV Bright Field was a bulk cargo ship that collided with the Riverwalk Marketplace shopping complex in New Orleans, Louisiana, on the afternoon of Saturday, December 14, 1996, after losing engine power. The vessel was fully loaded with grain at the time of the incident. The United States Coast Guard investigated the incident and published its findings on December 8, 1997, citing the cause of the engine failure as a poorly-maintained oil filter. A secondary but contributory cause was determined to be a main-engine automation system that produced warnings and alarms that were not consistently relayed to the ship's Master. More details
18. Harrier Jump Jets Disaster:
The Harrier, informally referred to as the Harrier Jump Jet, is a family of jet-powered attack aircraft capable of vertical/short takeoff and landing operations (V/STOL). Named after a bird of prey, it was originally developed by British manufacturer Hawker Siddeley in the 1960s. The Harrier emerged as the only truly successful V/STOL design of the many attempted during that era. It was a subsonic aircraft, unlike most of its competitors. It was conceived to operate from improvised bases, such as car parks or forest clearings, without requiring large and vulnerable air bases. 19. Marine Electric Disaster:
SS Marine Electric, was a 605-foot bulk carrier, that sank on 12 February 1983, about 30 miles off the coast of Virginia, in 130 feet of water. Thirty-one of the 34 crewmembers were killed; the three survivors endured 90 minutes drifting in the frigid waters of the Atlantic. The wreck resulted in some of the most important maritime reforms in the second half of the 20th century. The tragedy tightened inspection standards, resulted in mandatory survival suits for winter North Atlantic runs, and helped create the now famous Coast Guard rescue swimmer program. More details
